Cigarette & Tobacco Licenses
Overview
A Cigarette & Tobacco license is required for each establishment within the Town of Brookfield that intends to sell, exchange, barter, dispose of, or give away any cigarettes or tobacco products.
License Period
Annual; licensing period is July 1st to June 30th.
License Fee
$50
Application
Please begin with form CTV-100, linked below. Read the instructions carefully and include supplemental forms CTV-101 and CTV-102 if required for your business.
CTV-100: Retail License Application (Cigarette, Tobacco, and Electronic Vaping).
CTV-101: Individual Questionnaire (Cigarette, Tobacco, and Electronic Vaping).
CTV-102: Appointment of Agent (Cigarette, Tobacco, and Electronic Vaping).
Copy of Seller’s Permit Required
A copy of your current Wisconsin Seller’s Permit must be submitted with the application. The Seller’s Permit must be issued in the same name as the legal entity applying for the license. Applications submitted without the Seller’s Permit are incomplete and will be returned. Providing only the Seller’s Permit number does not meet this requirement. Training Required
Establishments are required to provide employees with tobacco sales training approved by the Wisconsin Department of Health Services. Please click here for additional information.
