Cigarette & Tobacco Licenses
Overview
A Cigarette, Tobacco and Vaping Device license is required for each establishment within the Town of Brookfield that intends to sell, exchange, barter, dispose of, or give away any cigarettes or tobacco products.
License Period
The licensing period is July 1st to June 30th annually.
Fees
License Fee: $50
Background Check: $20
Application
Please begin with form CTV-100, linked below. Read the instructions carefully and include supplemental forms CTV-101 and CTV-102.
CTV-100: Retail License Application (Cigarette, Tobacco, and Electronic Vaping).
CTV-101: Individual Questionnaire (Cigarette, Tobacco, and Electronic Vaping).
CTV-102: Appointment of Agent (Cigarette, Tobacco, and Electronic Vaping).
Renewals
Cigarette, Tobacco and Vaping Device renewals paperwork and fees are due 15 business days before the first Town Board meeting in June. When a licensed establishment fails to file renewal paperwork, this will result in a fee of $20.00 per day until the full application and fees are submitted.
Training Required
Establishments are required to provide employees with tobacco sales training approved by the Wisconsin Department of Health Services. Please click here for additional information.
